Family Partner, CEDARS Bridges Program

CEDARS Bridges Program provides supervised scattered-site apartments for homeless and state-involved youth preparing for independent living. Priorities are achievement of educational goals, increasing life skills, maintaining employment and development of a long-term supportive network. The Family Partner will utilize the wraparound philosophy and family-centered care to provide case management and coordination to youth residing in the Bridges Program. The Family Partner will work closely with team members including caseworkers from DHHS, Probation Officers from the Juvenile Probation Office and Life Skills Educators to support clients as they gain independent living skills. The general responsibility of the Family Partner is to develop and maintain an enduring relationship with assigned youth. The Family Partner shall be responsible for performing intensive targeted services to ensure that the agreed outcomes for the youth and their families are achieved. The Family Partner carries out assigned activities of the family, monitors and documents service delivery activities and outcomes, assesses and plans for the family. The Family Partner is the primary contact and resource for the assigned family, service providers and community supports for ongoing and crisis related care. Bachelor's degree in counseling, social work or closely related field and two years of experience working with children and families are required. The Family Partner must have an ability to use electronic data entry retrieval systems and effectively use computers. This position requires a valid driver's license, a good driving record and a reliable vehicle for use. Some cleaning tasks and moving of furniture in the apartments will also be expected. Flexible schedule with occasional evening and weekend hours to meet the needs of the youth. The Family Partner is required to participate in an on-call rotation and weekend curfew checks.
