The Family Partner (FP) provides primary case management functions using the Wraparound Philosophy and ensures that all parties are in good communication and share common understandings. The Wraparound process is an intensive, individualized care management process for families with serious or complex needs. The FP convenes a client and family team whose members consist of individuals agreed upon by the family and committed to them through informal, formal and community support and service relationships. The FP will build a trusting relationship with the family and ensure the development of the most relevant family goals, utilizing the family's strengths and the strengths and capabilities of the family team to achieve accelerated outcomes. The FP provides assertive leadership to coordinate the family-centered teams and acts as a liaison between the court and other formal systems and the family. The FP works directly with the formal systems to provide reports and information to help determine the most advantageous individualized service delivery combination. The FP is familiar with the varying backgrounds, sentiments and philosophies of the various systems and takes these into account in their assessment and service module decisions. The FP provides on-call 24/7 crisis management and intensive support. The FP shall have a Bachelor's degree in human services or closely related field and a minimum of two years of experience working with families in crisis. Knowledge and experience in case management is strongly desired. Applicants with diverse backgrounds are strongly encouraged to apply. This position requires a valid driver's license with a good driving record. Schedule: Flexible, 40hrs/week; typically between 10a-7p Monday through Friday to meet the needs of the family's schedules.
